Join us at United Way of Rhode Island for our Day of Action to sort and label donated children's books!

 Volunteers will receive United Way LIVE UNITED t-shirts and refreshments, including pizza, at 1:00 p.m.
T-shirts will be available at sign-in.
Community service letters can be provided upon request.

Please note: Children aged 16 and under must be accompanied and supervised by an adult.

 1. I UNDERSTAND THE NATURE OF THE ACTIVITY may involve risks, including minor or serious bodily injury. The type of injuries couldinclude tripping, injuries related to lifting, cuts, bee stings and loss of life, which may be caused by my own actions, or inactions, those ofother Activity participants, the conditions in which the Activity takes place, or the negligence of the ?RELEASEES? named below, and thatthere maybe other risks either not known to me or not readily foreseeable at this time. I acknowledge that if I believe any event condition isunsafe, I will immediately discontinue participation in the Activity.
 2. I FULLY ACCEPT AND ASSUME all risks and all responsibilities for any and all personal injury or property loss, that may happen during myparticipation in the Activity.
 3. I AM QUALIFIED, IN GOOD HEALTH, AND IN PROPER PHYSICAL CONDITION TO PARTICIPATE IN THIS ACTIVITY.
 4. I FULLY ACCEPT AND ASSUME ALL RISKS AND ALL RESPONSIBILITY for all personal injury or property losses, costs, and damages I incuras a result of my participation in the activity.
 5. IN THE EVENT THAT I REQUIRE EMERGENCY MEDICAL PROCEDURES I specifically authorize that all treatments be given. I also understandand agree that I will be responsible for the costs of such medical procedures and treatments.
 6. I HEREBY RELEASE, DISCHARGE AND PROMISE NOT TO SUE United Way of Rhode Island, its respective administrators, directors, agents,volunteers, and employees, other participants, any sponsor advertisers, nonprofit partners, and if applicable, owners and landlords of premises on which the Activity takes place (each considered one of the ?RELEASEES? herein), from all liability, claims, demands, losses, ordamages that I suffer which are caused or alleged to be caused in whole or in part by the alleged or actual negligence of the RELEASEES orotherwise, including negligent rescue operations.
 7. I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS EACH RELEASEE from any loss, liability, damage or cost which may incur, if, despite thisrelease and waiver of liability, and assumption of risk, I or anyone on my behalf, makes a claim against any RELEASEE.
 8. I HEREBY GRANT full permission to any and all of the above parties to use any photographs or other reproductions of this event for anylegitimate purpose including advertising.
 9. I READ THIS RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, INDEMNITY AGREEMENT FULLY, AND UNDERSTAND ITSTERMS AND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS by signing it, and have signed it freely and without any inducement orassurance of any nature. I intend to be a complete and unconditional release of all liability to the greatest extent allowed by law.
 10. I ALSO UNDERSTAND AND AGREE that United Way of Rhode Island may subsequently use, for publicity or promotional purposes, my nameor pictures of me participating in this event without liability or obligation to me.
